• fatigue
• carelessness
• improper behaviour
• improper dress
• alcohol and drug taking

Good topic Chrish!

It makes me think of the CAM theory

Most accidents that happen to humans are caused by

1)Conditions -what is housekeeping like-cables all over-tripping hazards

2)Attitudes-Fight with the better half-don't take it to work-you need to stay focused on the job

3)Methods-The way we do things-do we follow manufacturer's instructions or procedures recommended in their manual..
